Refer a Patient
As part of our commitment to children and their families, Blythedale places great importance on maintaining outstanding relationships with our referring hospitals and physicians. These relationships are vital to a continuum of care for our patients that helps to maximize their treatment gains and potential.
We are able to accept referrals to our inpatient hospital, Day Hospital program, and the Steven and Alexandra Cohen Pediatric Long Term Care Pavilion through Allscripts or via fax. Please include the following information:
- Family demographics
- Brief medical summary and recent medical progress note
- Recent medication list
- Current labs/diagnostics
- Physical/occupational/speech therapy notes (if applicable)
- Immunization record
- Insurance information (medical, no-fault if applicable)
