This program is directed to patients requiring the chronic use of medical devices such as ventilators, nasogastric or gastrostomy tubes, orthopedic or other devices. The inpatient interdisciplinary evaluation provides a time to assess outpatient medical, nursing, social service, equipment and other requirements for the safe discharge of patients with complex medical diseases and multidisciplinary needs.
