Referral Process

Patients admitted to Blythedale Children's Hospital have medical conditions of a complex nature and require a high intensity of care and services. Patients usually come directly from an acute care hospital, once they are medically stable.

Each referral is individually reviewed to determine if a child is an appropriate candidate for admission. The referral must contain both a medical and a psychosocial summary and insurance information. Criteria for admission vary with the medical program being considered. Medically complex patients need to be stable, yet require a level of care that justifies an inpatient hospital stay. Rehabilitation patients need to have the potential for functional improvement. Patients dependent on complex technology, such as ventilators, need to have caregivers willing to commit to learning their care.

Families are encouraged to visit Blythedale to see first-hand what Blythedale has to offer. Individual tours may be arranged by calling the Director of Social Work at ext. 462.

Once a child is accepted for admission, Blythedale staff will work with the referring hospital and parents to determine an appropriate time for admission. A parent or guardian must accompany each child to meet with staff and complete the necessary consent form. A copy of the child's birth certificate, immunization record, social security card and insurance card should be brought on the day of admission.
